This form helps us gather information so we can prepare your Wills or discuss your instructions with you.

This is a joint form for two people completing their Wills together. Some questions will ask for shared information, and some will ask for details about each of you individually.

In many cases, couples choose to make similar or matching decisions, but the form allows for different instructions if needed.

The questions are designed to collect information at a high level only. You do not need to provide exact details, and it’s fine to leave questions blank if you’re unsure.

Completing this form does not create a legal document and does not commit you to using our services. It simply helps us understand your situation and prepare for the next steps.

You can complete the form at your own pace. You may wish to go through it together, or one of you can complete it and the other review your answers before submission.

If you have children under 18, we will ask a small number of questions about guardianship later so that this can be addressed appropriately in your Wills.

If something doesn’t feel relevant to you, you can skip it and move on.
