Thank you for choosing Elephant Legal to help prepare your Lasting Powers of Attorney. This questionnaire gathers the information needed to prepare the LPA or LPAs you have chosen and to record your instructions, attorney arrangements and preferences clearly.
Before you begin
A Lasting Power of Attorney allows you to appoint people you trust to make decisions for you or support you with decisions if help is ever needed in the future.
Depending on your instructions, this questionnaire may cover a Property and Financial Affairs LPA, a Health and Welfare LPA, or both. It can also collect information for one person or for a couple completing up to four LPAs together.
Only questions relevant to your circumstances and chosen LPAs will appear. You do not need to have every detail decided before you start. Where you are unsure, you can say so and anything requiring further discussion can be dealt with before your documents are prepared.
Please do not enter passwords, PINs, bank security details, account access codes or other confidential login information into the questionnaire.
